By completing this form and submitting your information to the bank, you are accepting this agreement, you have elected to receive your periodic statements on account(s) provided to the bank during our verification process, and any required account disclosures in electronic format.
Our goal is to provide paperless statements, which are a great way to make your life easier, in addition to help saving our environment. No more waiting for your account information to arrive by the US Postal service, you can retrieve your statements any time after your statement date through online banking for up to two years. We will send an email to the email address you have provided to let you know your monthly statement is available. For accounts with multiple owners, only one account owner needs to enroll an account in the service. In accordance with the terms of the applicable account agreement and disclosure statement, all account owners will be bound by the decision of the account holder who enrolls in or cancels the service.
- You are solely responsible for promptly and timely sharing any account statement or any statement-related information with your joint account holder as the need may be and/or as requested of you by any respective joint account holder.
- Your statement will be available at the same frequency (cycle) as your statement that was previously mailed. You are responsible for accessing, opening, and reading your documents at your earliest convenience. These contain important and legally binding information and disclosures.
- You must maintain a valid active e-mail address; you are responsible for promptly notifying Garrett State Bank if your email address changes. Failure to notify us of changes to your email address could result in Garrett State Bank reinstating the delivery of your statement in paper format. If an email is returned undeliverable for any reason, we will try to contact you via phone at the phone number provided above, if the attempt is unsuccessful, we will resume the paper delivery of your statement via the US Postal service.
Please note that some deposit products may have a $2.00 per month paper statement fee. You can contact any branch to determine if this applies to your accounts. You understand that you have a duty to exercise reasonable promptness in examining the statement for any alterations, forgery, posting errors, etc. The statute of limitations governing these responsibilities will commence at the time the bank notifies you your statement is available.
We must hear from you no later than 60 days after we send you the FIRST statement notification on which an error or problem appears. You agree that Garrett State Bank’s security procedures are commercially reasonable. If at any time you wish to discontinue receiving your account statement electronically, please notify your local branch and we will convert your statement back to paper format.
At any time if you need a paper copy of a statement that you received electronically, you can call us at 260-357-0313 and request a copy. A $2.00 fee may apply.
To view your statements electronically, you will need to have Adobe Acrobat Reader loaded on your PC or another device. This free software can be obtained at www.adobe.com/reader/.
